Please make sure you have completed and included the following:

• A copy of your Drivers License, Social Security Card, any Professional Licenses (such as an A&P or FCC) and copies of all training certificates you have received.

• Background Verification form. You must complete and sign this form or provide us with a up to date copy of you resume (if you resume is not complete up to the present date you WILL BE REQUIRED to complete the background verification form. We need your work history for the last 10 years as well as two personal references.

• Request for Release of Records on Alcohol & Controlled Substances Testing form. Please complete Section One only! Leave Section Two blank. (To be completed by previous employer)

• If you requested a Direct Deposit form, please make sure that you mail the original form in with a voided check attached.

• The employee portion of the W-4 and I-9 forms must be completed and signed. Section 1 is the only employee part of the I-9. If you need to update you W-4 form or I-9 you can download these forms below and read them by using the Adobe Acrobat Reader.

• To be eligible for Per Diem, you must put your Per Diem address (must be over 50 miles away from the work site) on the W-4 and I-9 forms. In addition, make sure you complete the Certificate of Permanent Residence form.
